  I consent to the use of any videotapes and/or photographs in which my child may appear by the Diocese of Trenton and/or St. Paul Parish. I understand that these materials are being used for promotion of the children's choir's programs and/or activities, which may include recruitment and fundraising efforts.

  General Pick-Up Agreement
  When picking your child up from choir rehearsal, please pick them up in the lobby/vestibule of the church. If someone, other than the parent/s or legal guardian of the child will be picking up the child, please ensure that the choir director is given advance notice of this.
  
  Consent for Walk-Out Pick-Up
  
  As the parents or legal guardians of the above child(ren), I hereby give permission for my child(ren) to leave St. Paul Church when choir practice is over in order to connect for pick-up outside in the driveway or the parking lot, where I will be waiting.
  
  I hereby agree to waive all claims against St. Paul Parish and any of its employees/volunteers for responsiblity for my child(ren) once he/she leaves the building.
